Licensed Massage Establishment Renewal

Change of Name and/or Location

Florida passed House Bill 975 following the 2024 legislative session, which requires this profession to complete electronic fingerprinting. Your application cannot be approved until this requirement, along with all other licensure criteria, has been met. Before submitting a change of name/location application, be sure a change of ownership has not taken place. The following changes are examples of a change of ownership:

  • A license originally issued to an individual (sole proprietor), where the individual has changed.
  • A license originally issued to an individual (sole proprietor) which now operates under a Limited Liability Company or Corporation.
  • A license originally issued to a partnership where the partners have changed.
  • A license issued to a Limited Liability Company whose members have changed.
  • A license issued to a Corporation which has dissolved and reincorporated.

If a change of ownership has taken place, you will need to complete an Application for Massage Establishment License.

Change of Name

  • Fictitious Name Registration for any “doing business as” (D/B/A) name. If you provide a D/B/A and intend to advertise under a name that is not the name of your partnership, corporation or limited liability company you will be required to provide your registration.
    Fictitious name registration will be verified with the Division of Corporations.

Change of Location or Change of Name and Location

  • Fictitious Name Registration for any “doing business as” (D/B/A) name. If you provide a D/B/A and intend to advertise under a name that is not the name of your partnership, corporation or limited liability company you will be required to provide your registration.
    Fictitious name registration will be verified with the Division of Corporations.
  • Personal injury and bodily liability insurance for your establishment. You will need to submit a copy of your insurance documentation showing coverage at your establishment location to meet this requirement.

Change of Name Only

  • Submit your application, fee payment, and supporting documentation.
    Your application and fee payment may be submitted online, or by mail.

Applications submitted without fees will not be processed.

  • Submit any additional documentation, if required.
    If additional documentation is required, you will receive correspondence which includes a list of all items which are needed to complete your application. If you provided an email address, this correspondence will be sent to the email address on your application.

Once your change of name has been processed, you will receive an updated license by mail.

Change of Location or Change of Name and Location

  • Submit your application, fee payment, and supporting documentation.
    Your application and fee payment may be submitted online, or by mail.

  • Submit any additional documentation, if required.
    If additional documentation is required, you will receive correspondence which includes a list of all items which are needed to complete your application. If you provided an email address, this correspondence will be sent to the email address on your application.
  • Pass an inspection.
    Once your application is complete, staff will notify you that your establishment has been flagged for inspection. Ensure that the physical address, establishment name, and D/B/A of your establishment (if applicable) in the notification are correct. If there are inaccuracies, follow the directions in the notification.

Inspection timeframes may vary based on the location of your establishment. You do not need to contact board staff to schedule your inspection – a field inspector in your region will contact you at the phone number listed on your application.

Once your change of name/location has been processed, you will receive an updated license by mail.

You may not operate in a new location until your change of location (including inspection) is complete. Operation in a new location without completing your change of location constitutes unlicensed activity, which may result in citation, fines, disciplinary action against licensees working in the unlicensed location, and criminal penalties.

Change of Corporate Office/Interested Party/Designated Manager

  • A Designated Establishment Manager (DEM), who is a massage therapist with an active license without restrictions, who is responsible for the operation of the establishment in compliance with Chapters 456 and 480, Florida Statutes and Rule 64B7, Florida Administrative Code, if you are changing your DEM.
    Massage therapists who are also establishment owners may serve as the DEM of their establishment.
  • Background screening of any new establishment owner, completed through an approved Livescan provider.
    Massage therapists who are owners have already met this requirement in most cases; you may be required to complete a new background screening if the screening required to become licensed as a massage therapist was not retained.

Your massage establishment owners will be verified using your most recent filing with the Division of Corporations.

  • Submit your application and supporting documentation.
    Your application may be submitted online or by mail. No fees are required for this application.
  • Submit any additional documentation, if requested.
    If additional documentation is required, you will receive correspondence which includes a list of all items which are needed to complete your application. If you provided an email address, this correspondence will be sent to the email address on your application.

No fees are required for this application.

Renew Your License

Current licenses expire at midnight, Eastern Time, on August 31, 2027.

Failure to renew an active license by the expiration date will result in the license being placed in delinquent status. Failure by a delinquent licensee to renew within six months of the license becoming delinquent renders the license Null & Void without any further action by the board or the Department.

To ensure you receive your renewal notification from the Department, your current mailing address must be on file.

The Department will renew your license upon receipt of a completed renewal application and fees.

Additional Information:

  • The process of renewing a license and receiving a printed license in the mail may take 7-10 business days. Initiating contact to confirm the receipt of fees or the status of your license prior to this time will not expedite the renewal process.
  • You may renew online if you have a credit or debit card to complete the transaction. Please note that you can now renew online, print a copy of your application summary and mail it with your cashier’s check or money order to:

  • If you are renewing your license after the expiration date and your status still indicates Clear, you will be assessed a delinquency fee. The delinquency fee will be in addition to your renewal fees.
  • If your license was not renewed by the renewal deadline, it will become Delinquent. You may not operate a massage establishment with a delinquent license. Delinquent establishment licenses will be moved to Null and Void status if they are still not renewed 120 days after the renewal deadline. A Null and Void license cannot be reactivated. If you wish to be licensed in Florida and have a Null and Void license, you will need to reapply for licensure.
  1. Go to www.flhealthsource.gov and click the “Account Login” button.
  2. Click the “Returning Business Establishment/School” button.
  3. Enter your User ID** and Password**, then click the “Login” button.
  4. Confirm or update your MQA Services Account email address, then click the “Continue” button.
  5. You are now in the Dashboard. During renewals, a “Renew My License” option will populate no later than 90 days prior to your license expiration date. (Note: If you do not see the “Renew My License” option, please check back at a later time.)

**Please use the “Forgot User ID?” or “Forgot password?” link if you do not recall your user ID or password.

Certified check or money order should be made payable to the Florida Department of Health.

If renewing BEFORE your license expires, your fees will be:

If renewing AFTER your license expires, your fees will be:

$180.00

Licenses become Null & Void1 on March 1, 2026.

1Null & Void -The licensed practitioner failed to renew their licensure status for two renewal cycles, resulting in their license expiring.

There are no continuing education requirements for a Massage Establishment.
